The Village Charter School of Chester-Upland is excited to welcome you to the academic school year 2006-2007. The following items need to be presented as soon as possible.

◆ Transfer forms from other school (1st-8th Grade only)
◆ Report Card from prior academic school year
◆ IEP Record from prior academic school year
◆ Proof of Residency (Lease, Electric Bill, Water Bill, or Phone Bill)

◆ Application
◆ Birth Certificate
◆ Social Security Card
◆ Immunization Record
◆ Emergency Contact Sheet
◆ Emergency Contact Card
◆ Physical Examination Form
◆ Dental Examination Form
◆ Medical History Form
◆ Lunch Form
◆ School Network & Internet Policy Form

All information MUST be completed before the child STARTS SCHOOL.

Physical and Dental forms must be for the current academic school year.

Children who are 5 years old or will be on or before September 30th are eligible to enroll in our FULL DAY Kindergarten Program.

Stop by the Village Charter School any school day to register between the hours of 8:30 a.m. and 3:30 p.m. for the upcoming school year.
